Aspire Community Services (ACS) is seeking a compassionate, organized, and motivated Housing Case Manager to support adults with Intellectual and Developmental Disabilities (IDD) in obtaining and maintaining safe, stable housing. Through our Whole Person-Centered Care approach, you'll help individuals achieve greater independence, community inclusion, and an improved quality of life.

IMPORTANT: This is a primarily virtual position with required in-person travel. Applicants MUST live in Pennsylvania and have reliable transportation to travel to participant homes and other assigned locations throughout Pennsylvania. This is not a 100% remote, work-from-anywhere position.

What You'll Do

  • Conduct assessments to identify housing needs, strengths, barriers, and goals.

  • Develop individualized housing service plans using a Whole Person-Centered Care approach.

  • Assist individuals with IDD in locating and maintaining safe, affordable, and accessible housing.

  • Coordinate housing applications, rental assistance, public benefits, and community resources.

  • Collaborate with Supports Coordinators, healthcare providers, behavioral health professionals, and community agencies.

  • Advocate with landlords, housing authorities, and service providers.

  • Educate participants on budgeting, tenant rights, independent living, and community integration.

  • Conduct in-person home visits to monitor housing stability, participant well-being, and progress.

  • Complete quarterly home safety reviews and fire drills as required.

  • Travel to participant homes, community locations, meetings, and other assigned locations throughout Pennsylvania.

  • Respond appropriately to housing crises and emergency situations.

  • Maintain accurate electronic documentation, assessments, case notes, referrals, and progress reports.

  • Follow agency policies, HIPAA regulations, ODP requirements, and documentation standards.

Required Qualifications

  • Must be at least 18 years of age.

  • Must reside in Pennsylvania.

  • Must be willing and able to complete required in-person travel within Pennsylvania.

  • Valid driver's license, reliable transportation, and automobile insurance.

  • Ability to work independently in a primarily virtual environment while completing required in-person responsibilities.

  • Strong communication, organization, advocacy, and problem-solving skills.

  • Proficiency with computers and electronic documentation.

  • Ability to pass all required background clearances.

  • Commitment to supporting individuals with IDD.

Preferred Qualifications

  • Experience supporting individuals with IDD.

  • Knowledge of Pennsylvania ODP services, waiver programs, housing resources, and community supports.

  • Experience in case management, housing, behavioral health, or human services.

Education & Experience

One of the following is preferred:

  • High School Diploma or GED with 3+ years of relevant experience.

  • Associate's Degree or 60 college credits in a related field with 2+ years of experience.

  • Bachelor's Degree in a related field with 1+ year of relevant experience.

  • Master's Degree in a related field preferred.

Physical & Travel Requirements

Ability to sit, stand, bend, walk, and occasionally lift up to 25 pounds. Regular in-person travel within Pennsylvania is an essential function of this position. Applicants must be able to safely operate a motor vehicle and maintain reliable transportation.
